An exciting opportunity to join the Crisis Assessment and Response team as an Assessor has arisen.

The postholder will provide care to people who are in crisis to return to their usual level of activity (baseline) before requiring support. Where this is not possible the team will identify further support and/or long-term care needs and make appropriate referrals.

The postholder will also be required to line manage the Patient Support Workers.

The Crisis Assessment and Response team (CARe) is looking for Assessors to join our community based team.

The CARe team supports individuals in their own home, or usual place of residence with the overall goal of preventing hospital admissions and supports individuals who are in crisis to return to their usual level of activity (baseline) before requiring support.

The Crisis Assessment and Response team works alongside Oxford Health as part of the Urgent Community Response pathway and responds to urgent two hour referrals.

The team operates 7 days a week from 7am-10pm, this includes Bank Holidays.

The team covers the whole of Oxfordshire, so it is essential that the postholder has a full UK driving licence and a reliable car available for work.

The CARe team has the following two bases, Unipart Business House and Horton General Hospital. You will be assigned to the base closest to where you live, for mileage purposes but will be required to cover the whole of Oxfordshire.

The Trust comprises four hospitals - the John Radcliffe Hospital, Churchill Hospital and Nuffield Orthopaedic Centre in Headington and the Horton General Hospital in Banbury.

Principal Purpose
1. To be a member of the Service with a shared responsibility for assessing patients to avoid Hospital admission but whom are experiencing a Care/Support Crisis for their suitability to receive Service Support and thereby avoid an inappropriate admission to hospital.
2. To cover the Team Leader post in times of absence.
3. To complete review visits on a regular basis assessing individual patient needs and setting appropriate SMART goals. To complete all Service associated documentation associated with allocated patients.
4. To be responsible for identifying any changes and reviewing progress and updating care plans and SMART goals as appropriate.
5. To identify appropriate pathways for patients within the service and complete all necessary steps within the defined process - referring to other services as appropriate and completing all necessary paperwork.
6. To work under guidance of the Team Leader to formulate and adjust as appropriate, individual patient centred care plans and ensure that these are implemented by the Service
7. To line manage an allocated group of Patient Support Workers ensuring timely monitoring, supervisions and appraisals.
8. To deliver assessment and care alongside Patient Support Workers where appropriate including communicating with care agencies and other partners in accordance with Trust guidelines, confidentiality and with the consent of the patient.
9. To be able to function in a multidisciplinary team and be involved in supporting the Team Leader in the daily running of the service
10. To be responsible for the maintenance and development of your own Continuous Professional Development (CPD).

Specific Responsibilities

1.

Communication

1. Communicate and receive factual information regarding patients, their representatives and staff, using negotiation skills, reassurance, tact and sensitivity; and overcome any communication barriers.

2. Demonstrate the ability to evaluate and modify own communication style suitable for differing situations and the needs of individual, including listening, observing and interpreting.

3. Able to maintain records and documentation in accordance with local policies and the Data Protection Act and ensure that Patient Support Workers adhere to these standards.

4. To promote the profile of the Service to colleagues within the hospital and community including detailed knowledge of the referral criteria.

5. To ensure appropriate liaison with other agencies before and after the patient’s discharge from the service.
