As we evolve our structure to better serve our clients, we offer a completely digital telecom-system, making it possible for all of our vulnerable and elderly clients to feel safe in the comfort of their home, as well as in the outside world. We are looking for a Field Assessor, with experience of working in the care sector to join our amazing team in the Oxford area to facilitate Client reviews and referrals, installing assistive equipment and doing client assessments.
**What you will do as Field Assessor**:
- Be part of the Team to achieve the number of review visits to our clients, that is required to meet the Alert Service contract specification.
- Be part of the Team to complete referrals (and installations) with new and
existing clients county-wide (Oxfordshire).
- Identify the clients' current needs by completing appropriate paperwork and assessments,
in line with the Alert Service criteria and procedures.
- Work with the clients, their families, and third-party community services such as
GPs, nurses, social workers, and voluntary groups to maintain the independence and well-being of the client.
- Perform your duties within all aspects of the County Specification and your employment
contracts and job descriptions.
- Point the client in the right direction for advice, information, and support, and
ensuring preventative action by effective sign-posting.
- Maintain and develop effective working relationships with colleagues
across the Company so that a seamless service is provided to the customer.
- Provide updates as per Alert Service procedures, in particular, to inform the Resource Assistants who will update the database.
- Report any risk issues to the appropriate person, ensuring the health, safety, and security of the service user.
- Liaise with Lifeline services and respond to requests for assistance in a timely manner as per instructed procedures.
- Report faulty equipment promptly and to refer requests for additional sensors etc.
Maintain knowledge and expertise in specialist function and demonstrate commitment to own competence and professional development.
