Do you want to work in an integrated setting in primary care with colleagues from primary care, mental health services, including the voluntary and community sector, to improve access to support for people with mental health issues?

As part of our services across Oxfordshire we are implementing Primary Care Mental Health hubs to support GP practices with Mental Health assessment and support for patients in their communities.

As the lead clinician initially for one Hub you will work with a team of mental health professionals, Peer Support Workers, 3rdsector partners to provide the initial mental health intervention for 2-3 PCNs. You will provide triage, assessment and short-term interventions for people presenting for the first time with signs of mental illness, as well as for those with an SMI who are under the care of Primary care, offering early intervention and relapse prevention.

You will support the team to focus services on local communities and local GP practices to help rebuild and reconnect healthcare teams across the communities they serve.
