We are seeking a motivated and dynamic Band 4 Nursing Associate to work within our Rapid Assessment Team. The base of the position will be discussed at interview, however there is an expectation that staff will cover across the bases if needed. This is an excellent opportunity to develop your knowledge and skills within the community nursing setting, with the possibility of being supported to complete your Nursing qualification. You will have the opportunity to be involved in facilitating self-care for patients with long term conditions, health promotion and innovative activities and projects that enhance patient service delivery and staff wellbeing.

Experience in the field of adult community services is desirable but a willingness to learn and contribute is more important.

To provide, supervise, and monitor care and treatment to an identified group of patients under the direction of Registered Nurse. You will be part of a team of experienced nurses providing unplanned and urgent nursing care in community to help avoid hospital admission and ambulance conveyance by providing supportive care at home for the house bound population of Bedfordshire.

Youmusthave a driving license and have access to a vehicle in order to carry out your duties. You will be required to work a shift pattern covering 7 days a week from 08:00-21:00.

Take responsibility for the development and delivery of evidence based practice, ensuring that it supports and reflects service user individual needs, Trust objectives and professional guidance.

Contribute to the development of high quality evidence based in-patient services in a changing health care environment.

Contribute towards ensuring that the service functions as an integral part of the Directorate’s services.

Effective communication with all relevant personnel both internal and external to the Trust.

To practice within professional guidelines and promote professional practice in clinical area.

To ensure continuous personal development through the utilisation of regular supervision, annual appraisal and training where appropriate.
