This is an exciting opportunity for a Social worker, Registered Mental Health Nurse or Occupational Therapist to join our small, experienced team for a 12 month secondment
The key priorities for the team are;
To undertake a review of all Luton patients with SMI living in supported accommodation to help us to understand what sort of rehabilitation package of care might be needed to be to support move on and over what time period with a recovery focused pathway

As we move forward with the development of the NHS Long Term Plan and the vision for Community Mental Health, you will be at the heart of achieving the aspirations of the service users and carers we work with providing the opportunity to focus on people’s assets and strengths and to consider holistic, creative solutions to support the individual.

Identifying with the service user and the staff of the out of area placement clear goals for progression through the pathway being identified (e.g. managing medication more independently, self catering, budgeting).
Facilitating assessment by the potential move-on accommodation provider at an appropriate time.
Liaising with all parties, including family members/carers, and support the service user and family practically and emotionally through the assessment and move-on process, including visits, transitional leave and final move.
