**The opportunity**

Compton Care is looking to recruit a Palliative Care Coordinator to join the Care Coordination team.

The aim of the Care Co-ordination service is to provide advice and support for patients and their families/carers who are identified as in their last year of life. Care Co-ordinators will help patients navigate health and social care services and signpost to partner and voluntary organisations.

A further aspect of this role is to develop effective and constructive relationships throughout the wider palliative care community within Wolverhampton/Staffordshire in order to facilitate a clear and consistent approach to palliative care provision. This may include attending MDT meetings at GP surgeries, and promotion of the service.

**Essential Criteria**
- English GCSE level or equivalent
- Maths GCSE level or equivalent
- IT skills and Microsoft Office experience, including Word, Outlook, Excel, PowerPoint
- Previous experience of working in a healthcare environment
- Experience of managing own workload with the ability to determine those tasks that are urgent and require immediate action
- Experience of dealing with confidential and sensitive data and appropriate maintenance and storage of records
