Senior Crisis Practitioner

This service has recently gone through a process of review and change this post provides an exciting opportunity to be part of a flagship service offering community street triage assessments following referrals from emergency services and also to support individuals in the health based place of safety at the Becklin Centre who have subsequently been detained under s136 of the Mental Health Act. Part of the aims of the review were also to include more proactive work with individuals who are having multiple contacts with emergency services. This includes more work with other service providers and multi-disciplinary working and close link working with police, liaison and diversion services and the trusts multi-agency care plan coordinator. Relevant core professional nursing qualification (RMN/RNLD) Experience in working with adults with acute mental illness.
Ability to consult, liaise and negotiate with service users, carers/family members, other professionals and the wider community. Ability to work within a multi-disciplinary team. Able to demonstrate evidence-based practice. A commitment to the delivery of services that are responsive to service users, culture, gender and families Good written and verbal communication skills.
Able to prioritise work whilst working under pressure. Assess people at risk and seek appropriate advice and support. Able to delegate work whilst working under pressure. Demonstrate ability to act on own initiative.
Ability to work in partnership with other teams and services Effective team player A belief in recovery, psychosocial interventions, engagement and the involvement of service users and carers in their care. Can demonstrate a sound working knowledge of the whole system and relevant care pathways. A good understanding of mental health services, including relevant legislation (Mental Health Act, Capacity Act). A clear understanding and commitment to the role and purpose of Emergency Service Pathways (Street Triage and the Section 136 Suite) A clear understanding of the professional role, both profession specific and the generic nature of a senior crisis practitioner.
Understanding of the Care Programme Approach. A good understanding of the needs of people with mental health problems who are in acute crisis. Excellent assessment of clinical risk and experience of risk management Capacity to embrace change. Able to work across a full working week i.e.
7 days, 24 hours a day. Calm and confident with people who are in distress. Reflective and considered approach to problem solving. An awareness of the limitations of the role and when to escalate to senior staff.
