This is a unique and exciting opportunity to provide and support the highest standards of evidence based clinical practice within the newly reviewed and developed police pathway. This involves providing support to patients through the pathway from initial referral, community assessment and through the s136 detention process where applicable.

The Street Triage service commenced around ten years ago with the aim of offering mental health triage to people that come into contact with the police. The main aim of the service is to triage the risk and need of the patient, support the police in the use of section 136 and offer less restrictive interventions where this is deemed appropriate to do so. Street Triage also offer assessments of some patients in custody and support to the Ambulance Service when they encounter members of the public who have a mental health need, with the aim of diverting people away from the accident and emergency department.

We are looking for an experienced and enthusiastic mental health practitioner. A dynamic individual who has the ability to provide a quality service under pressure, able to work in a team and on their own with a specific regard to the interface between the criminal justice system, police, ambulance and all our partners.

You will be expected to offer expedient and sound information to professionals referring into the service. You will need triage and assessment skills as you will be communicating and providing interventions to individuals who might otherwise have been transferred to Emergency Department or detained on a Section 136 of the Mental Health Act.

Within the health based place of safety, you will need to be highly skilled in supporting individuals who may be distressed, experiencing acute symptoms of mental illness or intoxicated. You will be expected to have a sound knowledge of the mental health act.

The Leeds and York Partnership NHS Foundation Trust (LYPFT) is the main provider of Mental Health and Learning Disability services in Leeds. We also provide specialist inpatient services in York and some highly specialised services across the country. This service has recently gone through a process of review and change - this post provides an exciting opportunity to be part of a flagship service offering community “street triage” assessments following referrals from emergency services and also to support individuals in the health based place of safety at the Becklin Centre who have subsequently been detained under s136 of the Mental Health Act. Part of the aims of the review were also to include more proactive work with individuals who are having multiple contacts with emergency services. This includes more work with other service providers and multi-disciplinary working and close link working with police, liaison and diversion services and the trust’s multi-agency care plan coordinator.
- Relevant core professional nursing qualification (RMN/RNLD)
- Experience in working with adults with acute mental illness.
- Experience of supervising staff
- Experience in formulating care plans and safety plans for service users.
- Experience in conducting assessments within an acute mental health setting
- Assessment and problems solving skills, particularly in the area of crisis assessment and working with emergency services.
- Advanced level of skill in de-escalation of distress, and anger.
- Will know, maintain and constantly strive to improve, all standards of care.
- Ability to consult, liaise and negotiate with service users, carers/family members, other professionals and the wider community.
- Ability to work within a multi-disciplinary team.
- Able to demonstrate evidence-based practice.
