Mental Health Practitioner

The service has a proven track record in providing effective services to individuals with a history of trauma, self-harm and interpersonal difficulties. The service has now been adapted to better meet the needs of young people aged 18-25. EMERGE Leeds provides a range of interventions for people with complex emotional and relational difficulties including care coordination, Youth Work, Housing, Peer Support, Occupational Therapy, Psychotherapy and Creative Therapies. Senior Mental Health Practitioners sit at the heart of our established service providing case management and care coordination in the context of the multi-disciplinary team.
Mental Health Practitioners in the service manage a caseload and provide care coordination as part of a psychologically informed service, alongside colleagues from range of disciplines and partner organisations. The successful applicant will be expected to develop positive and meaningful therapeutic relationships with individuals who experience emotional distress and interpersonal difficulties, and to provide effective interventions and case management.
The Leeds and York Partnership NHS Foundation Trust (LYPFT) is the main provider of Mental Health and Learning Disability services in Leeds. We also provide specialist inpatient services in York and some highly specialised services across the country. Provide effective case management to caseload in the community.
Develop a therapeutic working relationship with service users whose difficulties have historically been described by the term ‘personality disorder’, and facilitate focussed treatment interventions as appropriate.
Collaborate with service users, carers and other service providers to develop effective care plans taking responsibility for implementation and regular reviews.
Ensure effective care co-ordination and communication within care teams, the wider Network and others involved.
Ensure that crisis plans are in place, up to date and circulated.
Undertake risk assessment and risk management for individual service users and to provide advice to other professions on risk assessment and risk management.
Undertake regular office duty
Attend monthly clinical supervision
Attend monthly management supervision
Attend weekly clinical meeting
Attend monthly Team Development sessions
Contribute to the collection of outcomes measures and take part in service evaluation
The service actively maintains a reflective, supportive and learning culture to support the work it does. Opportunities are provided to develop experience and skills and staff are supported to develop within their roles. Team members are provided with training and access to both regular clinical and managerial supervision.
You will be required to travel by car within Leeds on a regular basis.
