Care Coordinator

We are currently recruiting for a Care Coordinator to join our expanding team to perform a wide variety of duties in a timely manner to support the smooth running of the care operations aspects of the business. To support the key player team to perform their duties, by supporting client and care professional onboarding and quality assurance. This role is a combination of both field work and office based therefore, you will need to have a full UK Driving License and access to your own vehicle.
This role will be carried out on a full time basis 37.5 hour per week, Monday-Friday 9am - 5pm with a requirement for occasional out of hours working for visits, and as part of an on-call rota which attracts an additional payment to your basic salary.
Salary £24,000 to £27,000 depending on experience.
**The Role**
- Support the Registered Care Manager to respond promptly to care enquiries, assess the care needs of new clients and to develop care plans, risk assessments and visit schedules.
- Support the onboarding, and development of care professionals by carrying out supervisions, support visits and competency observation assessments in line with the care professional onboarding process.
- Carry out client care quality and service reviews as required to ensure good standards are being maintained.
- Maintain effective operational systems by ensuring that all client and care professional databases are kept up to date.
- Manage changes to care schedules and be prepared to cover gaps at short notice when care professionals are absent.
- Support the out of hours on-call function to ensure there is sufficient cover to deliver services and emergencies are responded to appropriately.
- Provide feedback to the Registered Care Manager, Scheduler, and the Recruitment Officer to ensure sufficient care staffing levels.
- Coordinate holidays for the office staff team.
- Support the recruitment and pre-employment checking processes for new Care Pros where appropriate.
- Coordinate Care Professional retention activities such as regular newsletters and recognition awards such Care Professional of the month etc.
- Support the Recruitment Officer to promote the business and advertise recruitment opportunities using social media and in conjunction with our PR partner.
- Carry out any other duties deemed necessary for the successful operation of the business.

**Qualifications** Essential Criteria**
- Good working knowledge of IT systems with experience of Microsoft Office and CRM software.
- A willingness to learn and adopt new technologies where appropriate.
- A practical, methodical approach to problem solving.
- Excellent organisational and planning skills with the ability to prioritise work tasks.
- A keen eye for detail and the ability to work accurately to deadlines.
- Self-motivated and able to work flexibly.
- Excellent written and verbal communication skills.
- Warm, friendly, and professional approach.
- Passionate about being part of a team, delivering outstanding care to clients.
- A willingness to develop a career in health and social care, which would include undertaking qualifications applicable to the role.
- A competent care at home professional with experience of working in the community.
- Due to the need to support the out-of-hours on-call function, you should have a driving licence and access to a vehicle as there may be a requirement to support team members out in the field in emergency situations.
