The post holder will provide a high quality based service to a defined group of frail patients and provide autonomous expert clinical care to patients presenting in the last year of life or who are requiring an MDT approach between the acute and primary care setting to ensure the patients care pathway is done collaboratively.

facilitation and delivery of Advance Care Planning for frail patients within locality for those individuals with and without mental capacity to support clinical decision making at end of life.

Increasing the number of Advance Care Planning documents in our patients medical records and most importantly ensure our patients voices are heard through referrals to the Advance Care Planning Program.
Provide information, guidance, support and referrals to patients and families regarding Advance Directives and in conjunction with the Advance Care Planning department. Advance Care Planning Facilitators engage individuals in thinking about and planning for their goals in the context of a serious illness or life limiting condition.

Provide ongoing clinical education to community members, cultural groups and healthcare providers to increase the awareness and development of patient’s choices and preferences regarding life sustaining treatment. Advance Care Planning Facilitators participate in business planning in a strategic level to support the continuation and future development of the ACP program through ongoing qualitative and quantitative reviews. Meet with patients and their agents to discuss values and beliefs regarding future health care wishes. Patient completion of the Advanced Directive naming agent and basic health care wishes.

- To work in conjunction with the Acute frailty Lead Nurse in the provision of a service to meet the demands of patients
- Demonstrated commitment to improving care for patients, including the seriously ill and those nearing the end of life. Knowledge of principles, concepts and methods of health care ethics.
- Strong interpersonal, verbal and written communication and advocacy skills required. Ability to work well with all disciplines on the health care team
- Act as a clinical role model, ensuring clinical expertise and practice is enhanced
- The post will have the skills and knowledge of frailty and end of life care
- They will be competent to prescribe or working towards undertaking the non-medical prescribing course.
- To clearly present the patients point of view to others, including promoting and protecting the interest and dignity of patients

Professional Duties
- To take an active role in risk assessment, supporting implementation of strategies to minimise risk. Ensuring incidents and near misses are reported, through promoting a no blame culture.
- To manage verbal complaints and refer on when unable to resolve, encouraging junior staff to be proactive in the management of verbal / potential complaints.
- To participate in investigation of incidents / complaints required, including preparing detailed report/ letters to complainants.
- To establish and maintain effective communication, both verbal and written and relationships with Multi-disciplinary team
- To recognise, prevent where possible, and manage areas of conflict, reporting to department co-ordinator/matron if unable to resolve
- Act as an innovative and enthusiastic role model, providing leadership, guidance and advice to staff on operational & professional issues
- Monitor and ensure standards of care are maintained and that policies/procedures and protocols are adhered to and updated where appropriate
- Ensure own practice is evidenced based and contribute to the development of nursing practice in own area, i.e. through essence of care
- Contribute to and participate in programmes of Audit, including utilisation of results in practice.
- Support the department co-ordinator/matron in aspects of Patient & P
