An exciting opportunity has arisen within CHFT frailty team. The Frailty Team Leader post is an
exciting opportunity to manage, develop and improve the journey of Frail patients who present
acutely to the hospital.
You will have line management responsibility for a team of specialist nurses, physiotherapists,
occupational therapists and coordinators. You will be supported by the matron for frailty to develop
and lead the team to provide high quality, safe and compassionate care for frailty patients.
This post would be ideally suited to an experienced, highly motivated and enthusiastic clinician. The
The successful applicant will have excellent communication skills and have an extensive networking
knowledge of staff and services related to acute frailty care.
- Provide leadership and management to the Frailty Team. Ensuring divisional and trust

compliance targets are delivered timely.
- Be proactive in the development of the frailty service, supporting the matron and lead and

nurse consultant in business planning process / service development, through contribution
of new ideas to improve patient care / experience.
- Contribute to investigation of complaints and incidents and subsequent action plans within

elderly medicine.
- Ensure adherence to all Trust-wide protocols, polices and directives within service delivery.
- Participate in service review and development alongside the Frailty leads and divisional

teams. Promoting and supporting the vision for frailty.
- To work closely with member of the Trust’s management team and other appropriate health

and social professionals to co-ordinate, influence and develop Older Peoples services within
and across the primary and secondary care settings, in line with local, network and national
guidance and standards.
- Work with other appropriate professionals to ensure holistic, comprehensive assessments,

care planning, care delivery and referral mechanisms are in place to support patients at
appropriate points of their care pathway, ensuring advice and information is available to both
patients and carers.
- To maintain and provide relevant statistics about activities as required by the Trust, network

and national organisations.

We employ more than 6,500 staff who deliver compassionate care from our two main hospitals, Calderdale Royal Hospital and Huddersfield Royal Infirmary as well as in community sites, health centres and in patients’ homes. We also are incredibly proud to have almost 150 volunteers here at CHFT.

We provide a range of services including urgent and emergency care; medical; surgical; maternity; gynaecology; critical care; children’s and young people’s services; end of life care and outpatient and diagnostic imaging services.

We provide community health services, including sexual health services in Calderdale from Calderdale Royal and local health centres. These include Todmorden Health Centre and Broad Street Plaza.

We continue to modernise and invest in our health services to build on our strong reputation. Foundation trusts are public leaders in improving quality in health services. They are part of the NHS - yet decisions about what they do and how they do it are driven by independent boards. Boards listen to their Council of Governors and respond to the needs of their members - patients, staff and the local community.

Foundation trusts provide what the health service wants, yet are also free to invest quickly in the changes to the local community needs, in striving to be the best, and in putting their patients first.
- To develop a highly specialist service for patients living with frailty.
- Actively collaborate with the wider multi-disciplinary team in order to lead the development of

a strategy relating to the people living with frailty.
- Lead in the development of the frailty service, having a clinical understanding of each role

within the MDT and proactively ensure each member has a voice within the team..
- Act as a highly specialist adviser to the hospital, community and other agencies as required.
- Work collaboratively with clinical leads/ specialist teams in the community.
- Maintain an active role in user involvement.
- Develop, implement and evaluate teaching programmes for both multi-disciplinary team/other

healthcare professionals (including formal/informal post graduate teaching to doctors and
pharmacists) and for patients living with frailty.
- Line management and development of the specialist team. Ensuring divisional requirements

and standards are met.
- To work in partnership with the multi-disciplinary members of the Frailty team to provide a

Comprehensive Geriatric Assessment (CGA) for patients living with frailty and presenting in an
acute setting.
- Provide the core components of the clinical specialist, which includes: clinical practice,

management and education.
