Living Well Older Adult Mental Health Link Worker

The national strategic direction in General Practice (within Primary Care Networks) is to employ via the Additional Roles Reimbursement Scheme (ARRS) a wide range of health and care professionals to manage the increasing demand via the ARRS scheme.
As part of this national scheme, we are looking to recruit older adult mental health link workers (social prescribers) to work with adults over the age of 65 with functional mental health issues across all Wigan Borough PCNs with the ambition that it will support joint working with local mental health services and improve access to appropriate community services.
In this role, you will support older adults within a primary care network setting to enhance their health, wellbeing, and quality of life, assisting them to connect with and access mental health services, local authority services, voluntary sector services and community-based activities and support. The role will be delivered through working as an integral part of both primary and secondary care services, to connect individuals to appropriate support at the earliest opportunity.
You will be supported by members of the local GP practice, wider primary care network and Living Well team. CPD opportunities are available and personal interests/skills will be supported to develop and grow. You will work alongside adult mental health link worker and community link workers involved in social prescribing within the Primary Care Network.
The role will be based in GP surgeries and working in the local community linking in with health, social and VCFSE organisations. For a detailed description of the main duties see job description.
