The post holder will be expected to work autonomously and as a member of the community forensic service supporting the Team Manager in ensuring oversight of the case management of the TC forensic cohort. The individual well be expected to work closely with teams across the LPFT footprint, Lincolnshire County Council Social Work teams and other Mental Health Services/Specialist Providers to support discharge for forensic individuals in inpatient settings, provide intensive support in the community and reduce inappropriate admissions to hospital settings.
The post holder will be based in Lincoln but work across the county and will be expected to work flexibly Monday to Friday between the hours of 9am to 5pm. There may also be travel regionally as required to out of area provider placements.
The post holder will provide oversight of the individuals with Intellectual Disability and/Autistic who fit the Transforming Care Programme as part of a funded parallel multi-disciplinary sub-team within an existing generic Community Forensic Service.
The role requires a high level of clinical and professional expertise in order that Individuals with Intellectual Disability and/or Autistic Individuals benefit from a well-coordinated pro-active, specialist, efficient and effective multi-disciplinary provision. The service includes providing wider development, clinical leadership, clinical team formulation, individual assessment, therapy and management, training, advice, and a consultative approach. The focus of all support and intervention is to adhere to the ‘Building the Right Support’ Principles for this cohort of individuals within the NHS Long Term Plan - maintaining and supporting individuals to live well in the community, by reducing inappropriate hospital admissions and working to prevent placement breakdowns.

**Summary below**:
The transforming care part of the overall forensic caseload (around 16 individuals) will include patients in hospital who are transitioning to the community, as well as community-based patients.
The Specialist Practitioner will take an important role in overseeing the work with this cohort, leading along with the Clinical Psychologist in ensuring the service is providing an intensive multi-disciplinary service.
To attend Transforming Care meetings/forums both internally and externally to represent CFS and provide timely and accurate updates on the case management of TC cohort.
To be accountable to the CFS team manager and LPFT TC Lead to the progress of the service in supporting TC cohort and in ensuring identified actions are carried out removing barriers in care provision.
To support wider CFS team members in the case management of TC cohort and enabling required and necessary access to specialities is avaialble ie; OT, SLT, PBS, Psychology, Psychiatry
To ensure those in OOA placements are managed and team members are actively involved in discharge planning including site visits as required
