Working as part of a multidisciplinary team to improve the quality of care for homeless hospital patients through providing specialist advice and advocacy you will work to support quality discharge interventions with a focus on accommodation options.

We are seeking an enthusiastic, empathetic individual who has a clear understanding of professional boundaries. Ideally you will have experience of promptly assessing, planning and completing actions with Homeless and/or vulnerable adults, in very complex situations.

You will take a person-centred approach to address the issues faced by homeless or vulnerably housed people and the difficulties experience in accessing services.

You will maintain a caseload of clients, offering them assertive, consistent ongoing guidance and support until they are in a position to be able to move from the street into accommodation and/or other

**Essential Requirements**:

- Must have working knowledge of the legal framework governing homelessness and allocations
- Knowledge of the current housing market and the environment in which the local authority operates in
- Experience of partnership working with healthcare services, or working in healthcare settings
- Understanding of safeguarding of vulnerable adults principles and procedures
- Knowledge of local and national agencies who can assist our customers
- Must have a complete and thorough understanding of the housing options available to people
- Must have excellent interpersonal and negotiation skills both on the phone and face to face
