We are looking for a **Care Coordinator **to join our team in West Bridgford, on a part time basis - around 10 hours per week of Care Coordination duties and the rest made up of Care Work. In return you will receive £24,500-£25,500 per annum (pro rata for part time and depending on qualifications and experience).

This position will be part time Care Coordination with the remaining hours supplemented by delivering care outside of your Care Coordination hours.

**As our Care Coordinator, you will**:

- Ensure seamless, organised and effective scheduling and care delivery
- Review care plans
- Be instrumental in the onboarding of new Clients
- Supervise the day-to-day community operations, whilst managing the community care team.
- Carry out on-call duties outside working hours as agreed, be willing to step in to cover emergencies, and ensure regulatory requirements are met
- Deliver care when required to meet the needs of the business

This role is about more than just rota management. You will be responsible for supporting the Manager with Business Growth and ensuring we are always delivering the highest possible standards of care.

**Skills and experience of our Care Coordinator**:

- Full UK driving license with access to a car is essential;
- NVQ Level 2 in Health and Social Care and looking to work towards an NVQ Level 3
- A minimum two years care experience
- Good people management skills
- Able to work as part of a team as well as under own initiative
- Highly organised and able to prioritise
- Must have Good IT skills and know how to use Outlook, Word and Excel
- Good oral and written communication skills
- Knowledge of domiciliary care provision and CQC regulations is essential

- Rushcliffe, Nottinghamshire
- Beeston, Nottinghamshire and surrounding area

**Full UK driver's licence, own vehicle and flexible availability is essential.**
