We are looking for a professionally qualified (RMN, SW, OT)and suitably experienced band 6 Care Co-Ordinator with experience of working with young people to join our vibrant specialist CAMHS psychosis service. The team works collaboratively with individuals and their families who experience first episode psychosis within a bio psychosocial / spiritual and recovery-based approach.

We work in a multidisciplinary team approach and with excellent formal and informal support to help you manage your workload. Caseloads are set at manageable levels and we offer regular clinical and management supervision along with developmental opportunities relating to our specialist approaches e.g. cognitive behavioural therapy, family interventions, physical health, including various in house skills based training to include vocational and psychosocial interventions.

Extensive regular travel across the district is a requirement of the role in order to successfully undertake the range of duties specified within the job description. Therefore, it is an essential that applicants have the ability to travel regularly across the district to meet the needs and demands of the service.

Accountable for the co-ordination of care plans for people who are in the early stages of psychotic illness between the ages of approximately 14 - 18yrs. Joint working with members of the CAMHS & Adult Teams where appropriate.

Assess health and social care needs and negotiate care plans with young people and their parents/carers, remaining objective when working with diagnostic uncertainty.

Deliver and administrate care plans in partnership with multidisciplinary colleagues and a range of health, education and social care agencies.

Engage young people and their parents/carers assertively and offer care/interventions within the least restrictive setting in normal community environments.

To develop and maintain therapeutic relationships with young people and their families and carers who may at times be in situations of emotional distress.

To provide an age, developmental, culture and gender sensitive service.

Offer a range of evidence based interventions and proactive risk management approaches to help the young person and their family to reduce and manage symptoms, high risk behaviour and other effects of psychosis and enable, as far as possible, full and sustained recovery.

To ensure Bradford and Airedale Child Protection Guidelines and procedures are fully understood by self and others. To ensure the safety needs of young people are met at all times and take appropriate action when a young person is felt to be at risk.

Monitor treatment, support concordance and ensure prompt action is taken to alleviate unwanted effects.

Provide access to and ongoing psycho-education regarding first episode psychosis to young people and their families/carers.

Develop and maintain effective working relationships and networks with colleagues within mental health services, primary care, education and other partner agencies

Ensure basic needs are addressed either directly or through referral i.e. housing, accessing education/training, social activities etc.

Recognise the developmental needs of young people referred to the service and enable a vocational/educational assessment within a specified time of their contact with the service.
