We are looking for a highly motivated individual to coordinate both our Dynamic Support Register and our Care (Education) and Treatment Reviews within the Bradford district, specifically, adults with a learning disability and/or autism.

You will be working as part of the Intensive Support Team based at Waddiloves Health Centre.

The Intensive Support Team includes health care support workers and CPA Coordinators, whose over arching aim is to provide crisis support and advice in order to prevent the detention of adults with a learning disability under the Mental Health Act.
