**Main duties of the job**
- To work as a Care coordinator within WISHH & Five Lane Ends PCN to carry out a proactive comprehensive geriatric assessment on a cohort of patients with moderate frailty using a risk stratification tool, and for patients referred to PACT.
- Care coordinator to work with other members of PACT MDT to visit patients on a proactive or reactive need.
- Care coordinator to work with PACT MDT on Enhanced Health in Care homes. Sharing CGA skills to complement Care home personalised plans.
- To undertake a comprehensive geriatric assessment and document on the clinical reporting system. The assessment will be carried out by the Care coordinator with support and supervision from the wider PACT MDT as needed.
- Contribute to the week PACT MDT present patients for discussion/feedback

**Overview of your organisation**

The WISHH & Five Lane Ends PCNs consist of six practices in total covering a total population of over 70,000 patients. WISHH PCN consists of two GP Practices - Idle Medical Centre & The Saltaire & Windhill Medical Partnership. Five Lane Ends PCN consists of 4 GP practices - Moorside surgery, Ashcroft surgery, Farrow Medical centre & The Rockwell & Wrose practice. We have a history of working together to build a better service for our population, ensuring services are safe, accessible and of the highest quality. As both PCN’s work closely together we are finding innovative ways to ensure we share and spread success, sharing management resource and project management capacity. This shared approach is allowing us to rapidly build on the pro-active care model we have been developing as part of the Care-Coordination ‘’Act as One’’ programme in Bradford District and Craven. We have already been recognised for our innovative integrated approach and celebrated at the Act as One festival: link here. We are also currently sharing case studies with NHSE and the HSJ as an example of good practice in utilising the PCN DES contract innovatively.
