Patient Care Delivery1. Development of care plans related to discharge and discharge planning2. Develop information packages for patients on the discharge process3. Assist in the teaching or facilitation of teaching patient related skills to care staff4.

Responsible for facilitating nursing assessment forms are completed promptly and to the required standard for those patients whose cases are going to panel. Coordinate the completion of assessment forms from other disciplines. Ensure that parent and carers are supported through each phase of this process.5. Ensure that all patients and families are treated in a calm, courteous andreassuring manner at all times.6.
