Discharge Coordinator

University Hospitals Sussex NHS Foundation Trust Brighton (UHSx) is the regional teaching hospital working across two sites: the Royal Sussex County Hospital (RSCH) in Brighton and Princess Royal Hospital (PRH), Haywards Heath.
This is an exciting opportunity to be part of an Integrated Discharge Team (IDT) at the RSCH working alongside an energetic, motivated health care professionals who have a drive to deliver the best possible discharge outcomes for our patients.
The Discharge Teams are going through an exciting period of development and change, We now have a Discharge Hub at the RSCH, which coordinates the Discharge Flow 7 days a week. We work closely with the community providers in order to deliver on prompt and safe discharges.
The role will help the flow of patients through the hospital but also into the community. It will include;
- Liaising with patients, families and carers
- Liaising with nursing staff and community services
- Completing referral forms for the three CCGs areas (Brighton and Hove, East Sussex and West Sussex)
- working very closely with ward staff and Doctors
- Setting up/ attending meetings as directed
- Using IT skills to file and record documentation
- Completing Fast Track referrals and funding forms
The operational hours are Monday - Sunday from 0800-1800.
Supporting with discharges for all the pathways (0, 1, 2 and 3)
working in collaboration with all key professionals
supporting wards with their referrals
Assessing and making discharge plans for all complex patients
supporting with flow in the hospital

Act as patient /carer advocate at all times
To follow the trusts discharge policy and processes
To act as a key contact with partner provider organisations in ensuring pathways both to and from services work smoothly and prevent delays in transfers of care
To work in collaboration with the Discharge Hub based at RSCH in relation to any discharges.
Ability to act as discharge coordinator lead in a defined clinical care area to ensure safe and high quality discharge practice is undertaken.
Required to use own initiative, take responsibility for decision-making and prioritise own workload within a team context.
