If you share our values, then we want you to join CWP’s 18-25 consultation & advice service as a clinical lead covering the CWP trust footprint.
This specialist service was developed to provide a more comprehensive and joined-up offer for young people with mental health needs across the trust. We provide a senior clinician led consultation & advice clinical model which is available to all CWP services supporting young people aged 17 to 25. The service supports the existing multi-disciplinary team in formulating an understanding of the young person’s biopsychosocial needs, supporting decision making and planning, and helping to identify pathways for their ongoing care. In particular, we support teams working with young people with high complexity, whose care pathway is not clearly defined and where the specialist consultation & advice from 18-25 is indicated.
support in delivering high quality, trust wide mental health consultation and advice to teams that access the 18-25 service
take a holistic view of a young person’s care, whilst considering their strengths and aspirations.
support with the navigation of health, social care and educational services and assist with reducing barriers to accessing services.
regularly meet with a broad range of teams, triage referrals and utilise their expertise within consultations (alongside other members of the team).
work autonomously within a multi-disciplinary framework, providing specialist assessment and clinical support.
CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care - including the provision of three GP surgeries.
