Access Team to act as the first point of contact relating to access management, referral to treatment and all wait time targets. Access Team to act as the first point of contact for patients exercising their legal right to access services within maximum waiting times under the NHS Constitution guidelines. Direct liaison with patients as part of the national and local waiting list initiatives. Liaison with patients with deteriorating or changes of condition, assessing whether a clinical review is required, and following up with the relevant trust specialty to ensure patient best interests are adhered to.

To be a role model demonstrating good communication and ensuring robust and open communication with all staff through regular meetings with the specialty areas. In conjunction with the Access Co-Ordinators work with specialty management teams to ensure patients waiting for appointments and/or treatment are managed in chronological order, within clinical priority and seen or treated within the maximum wait time in line with national guidelines and Trust policy. To track, monitor and record progress against identified measures, including demand, capacity, recovery, and activity plans, to enable reporting on a regular basis. To lead on the development and delivery of training to individuals and groups of staff to support new and existing processes which support the Access Policy.

To lead on the Access Policy Audit programme to ensure compliance, providing feedback on waiting list management and recommendations for continued improvement. Co-ordinate highly complex waiting list validation programmes and manage outcomes, and reporting to National teams as required to validate data and reduce patient waiting times. Lead the work on capacity and demand within the Access Team to ensure ongoing maintenance and use in planning operational delivery. To support the electronic patient record programme by contributing their knowledge of RTT and elective care pathways.

Identify, recommend, and facilitate change to improve access by focusing on challenging areas within the patients pathway.To analyse, interpret and make recommendations on a range of factors relating to: Data quality Data completeness Capacity & demand To provide general administrative support including minute taking, arranging meetings, action plans and updating reports. To respect the confidentiality of all matters relating to their employment and other members of staff. Weekly monitoring to include (list not exhaustive): Not added to BEA All waiting list types Post inpatient & outpatient follow-up Incomplete > 18 weeks validation Pathway ID Unconfirmed TCIs Same day cancellations to ensure patients are re-booked within 28 days or within RTT breach date (whichever is sooner). MAXIMS internal referrals RTT data quality issues (Radar) Mutual aid monitoring Identifies and evaluates areas for potential service improvement and discusses with relevant personnel which may be from other areas of the Trust and impact on their working practice.

Encourage and enable others to alter their practice in line with service improvements.
