Liaising with other teams within and external to Forward Leeds, other professionals, statutory and non-statutory bodies. To work effectively within the multi-disciplinary team -To develop and maintain excellent links with other agencies so as to ensure effective quality care for service users. -Able to Communicate with service users who may be distressed and who may, on occasion, present with challenging behaviour. - Advise healthcare professionals on the management of patients with co-existing physical health an substance misuse problems - To ensure safe practice and ensure that the service is effective and safe for service users, staff and visitors.
- To use clinical and line management supervision, as well as ad-hoc supervision if and when required, supporting decision making and managing complex situations. - To report incidents in a timely manner using the Hub reporting system in line with Forward Leeds and Trust policies. - Provide specialist clinical expertise with the provisions of assessments, care planning evaluation, risk assessment and follow up to patients. - Manage a caseload and also deliver psychosocial interventions to service users within the wider service.
- Deliver a range of evidence led psychosocial interventions and support pharmacotherapy as appropriate with support from senior staff. - Perform physical health checks and interventions such as venepuncture, immunisation, wound care, urinE sampling. -Participate in clinical supervision based on video recording of practice.
