We are currently recruiting for a Complex Care Manager for one of the UK's leading complex care at home providers, working partly remote with teams operating in the **Bolton and Preston**areas.

Joining this large community complex care provider, you will be responsible for coordinating a team of support workers in the delivery of complex care packages within the community.

**Purpose of the role**:

- Coordinate the set-up of a new client package, including undertaking the initial assessment meeting with the client to determine their needs. (Upon either on discharge from hospital or transfer from another provider).
- Prepare monthly rotas in a timely manner for the care assistants and support workers, and ensure all clients receive service as designated in their care plan.
- Build and maintain effective working relationships with key members across the business, including the OM, clinicians, other CCM’s within the service and direct reports.
