Advanced Care Practitioner

As an Advanced Care Practitioner, you will be considered a key person in day-to-day clinical leadership and expertise within the frailty team. The Advanced Care Practitioner will demonstrate highly developed specialist knowledge and skills in the management and care of patients living with frailty. You will work autonomously within your sphere of competency as a clinician in a diverse multi skilled team which includes Medical & Nursing teams both internally and within our community Trusts, Occupational Therapists, Pharmacy leads and administrative staff. The Advanced Care Practitioner will formulate clinical decisions and treatment plans to manage the patient illness and injury. They will be part of the wider frailty team and will participate in the development of the service by working with our colleagues in the Emergency departments to pull through patients that meet the frailty SDEC criteria and work to ensure these patients are given specialist care so that they can return to their place of residence as soon as possible. They will carry out holistic needs assessments of patients, initiating interventions and referring to members of the multidisciplinary teams as appropriate. They will exercise advanced clinical expertise, levels of judgement for the development and review of care protocols and training for staff to further enhance and develop out frailty service.
Managing own caseload, carrying out comprehensive assessments, being professionally accountable and legally responsible. Assess, evaluate and implement plans of care for patients including their clinical treatment plan. Stream patients to expedite discharge to prevent unnecessary admissions and prevent deconditioning. Be a specialist in the field of frailty, and advise/review as required.
