Advanced Care Practitioner

Maintain effective professional and productive communications with patients, relatives, the multi-disciplinary team, external agencies, demonstrating advanced communication skills. Communicate highly complex or contentious information with sensitivity, tact and diplomacy. Communicate potentially distressing information and diagnosis to patients and relevant others as appropriate. Keep accurate and complete records of all activities and communication consistently with legislation policies and procedures.
Undertake investigation and documentation of a range of adverse or untoward incidents. Refer appropriately on to other members of the multi-disciplinary team as required. Be able to identify and take into consideration barriers to understanding and employ appropriate methods to ensure effective communication particularly where barriers are clinical in nature Provide emotional support to patients and carers Be aware of the procedures to follow if concerned regarding the protection of children and vulnerable adults Ensure compliance with policies, procedures and clinical guidelines for self and others Service Delivery and Improvement To develop and implement plans, processes and internal policies to ensure that the Trust conforms to the requirements of relevant legislation Responsible for interpreting, proposing and drafting changes to policies, guidelines and service level agreements (SLAs) that impact the service Proposes changes to own function making recommendations for other service delivery Maintain a good knowledge of emerging policies from government departments and external bodies which may impact the service Contribute to strategic and contingency planning for the service which may have an impact on other areas Proposes policy or service changes Participates in reviews of work practices which impact on service delivery to patients/clients and other specialities People Management and Development To adhere to equal opportunities and all HR policies To provide effective leadership across the service areas To be a role model, this underpins the corporate values and behaviours agreed by the organisation To ensure professional standards and practices are maintained by creating and supporting a culture where healthcare teams and individuals evaluate the work they undertake, celebrate success and learn from mistakes Time management skills To lead, motivate and support team members in the facilitation of professional and clinical development. Participate actively in the in-service training and provide teaching through mentorship and clinical supervision within the specialty.
Provide operational management and clinical leadership for the team including punctuality, shift allocation and rest breaks and triage. Respond to immediate sickness absence (requesting Bank staff, as per protocol as required) and ensure efficient and effective use of resources and estates policy. Patient Care Delivery Deliver specialised, evidence-based advice suitable for the individuals medical, social, psychological, religious and cultural requirements Perform assessment of patients needs, diagnose where appropriate, plan, implement and evaluate clinical treatment and care using advanced clinical knowledge and skills Promotes peoples, equality, diversity and rights Learning and Development Attend mandatory training updates as required. Critically assess and address own learning needs, negotiating a personal development plan that reflects the breadth of ongoing professional development across the four pillars of advanced clinical practice Engage in self directed learning, critically reflecting to maximise clinical skills and knowledge, as well as own potential to lead and devlop both care and services Engage with appraisal and repsonds to individuals motivation, development stage and capacity, working collaboratively to support healthy literacy and empower individuals to particpate in decisions about their care and to maximise their health and well being Faciliate colloraboration of the wider team and supports peer review processes to identify individual and team learning Identify further devlopmental needs for individuals and the wider team and supports to address these Support the wider team to build capacity and capability through work based and inter
