Salary:
£15.00 per hour/ £29,241 per annum

We are seeking a dedicated individual to join our team in this exciting new role.

You will provide a tenancy support and intensive housing management service for up to 20 properties around Rochdale, Heywood and Middleton and will need to be able to travel between these sites. You will be responsible for case management of people being released from hospital under the discharge to assess pathway. By removing barriers and ensuring reasonable adjustments are made, you will ensure people can move on into more appropriate housing arrangements.

**About you**
- Background and experience in support/social care or other relevant role
- keen to make a difference to people's lives by supporting those who have been discharged from hospital
- Excellent communication skills and an empathetic nature
- Able to work flexibly in accordance with the needs of the service

**About the role**
- Responsibility for housing management functions across up to 20 homes
- Support of people discharged from hospital who are unable to return to their own home
- Enable these beneficiaries to increase their independent living skills, assisting with access to education, employment, training and voluntary work to achieve their full potential
- Participation in staff development & training and support the overall ethos of the Trust
