**Job Summary**:
Working within own professional and clinical boundaries, the PCN Cancer Care Co-Ordinator will support GP practices within the PCN to deliver the key objectives of the Primary Care Network DES and to deliver timely and personalised care for patients as part of an established multi-disciplinary team. This post will particularly be supporting the early cancer diagnosis and cancer care quality improvement work by supporting practices to improve their processes, achieve their targets and working with patients to help them ensure they have the right support at each stage of their journey.

**Duties & Responsibilities**:
Following PCN policies, protocols and guidelines, duties will include:

- Supporting practices to deliver quality improvement plans for early cancer diagnosis
- Develop and embed systems across the PCN to improve cancer screening uptake, liaising with external agencies as appropriate
- Proactively identify and work with patients newly diagnosed with cancer and on the cancer register to deliver personalised care
- Ensure patients receive a Cancer Care review in line with national defined timescales and targets
- Support practices in conducting peer to peer learning events that look at data and trends in diagnosis across the PCN, including cases where patients presented repeatedly before referral and late diagnoses
- Support patients to utilise decision aids in preparation for a shared decision-making conversation
- Holistically bring together all of a person’s identified care and support needs, and explore options to meet these within a single personalised care and support plan (PCSP), in line with PCSP best practice, based on what matters to the person
- Help patients manage their needs through answering queries, making and managing appointments, and ensuring that patients have good quality written or verbal information to help them make choices about their care
- Support patients and carers to take up training and employment, and to access appropriate benefits where eligible
- Support patients to understand their level of knowledge, skills and confidence (their “Activation” level) when engaging with their health and wellbeing, including through the use of the Patient Activation Measure
- Assist patients to access self-management education courses, peer support or interventions that support them in their health and wellbeing and increase their activation level
- Explore and assist patients to access personal health budgets where appropriate
- Provide coordination and navigation for patients and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches, and other primary care professionals
- Support the coordination and delivery of MDTs within the PCN
- Work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients, and where required and as appropriate, refer people back to other health professionals within the PCN
- Raise awareness within the PCN of shared-decision making and decision support tools
- Raise awareness of how to identify patients who may benefit from shared decision making and support PCN staff and patients to be more prepared to have shared decision-making conversations
- Safeguard patients by ensuring organisations and groups have basic safeguarding processes in place for vulnerable individuals and provide opportunities for the patient to develop friendships and a sense of belonging, as well as to build knowledge, skills and confidence

**Other responsibilities within the PCN**:

- Understand, and contribute to, the PCN strategic plan
- Improve quality within limitations of the service
- Understand the implications of national priorities for the team and/or service
- Demonstrate understanding of, and conforms to, relevant standards of practice
- Demonstrate ability to identify and resolve risk management issues according to policy/protocol
- Work closely, and in partnership with, the Social Prescribing Link Worker(s) or social prescribing service provider and Health and Wellbeing Coach(es)
- Recognise the roles of other colleagues within the PCN and their role in patient care
- Demonstrate use of appropriate communication to gain the co-operation of relevant stakeholders (including patients, senior and peer colleagues, and other professionals, other NHS/private organisations e.g. CCGs)
- Demonstrate ability to work as a member of a team
- Recognise personal limitations and refer to more appropriate colleague(s) when necessary
- Liaises with other GP Practices and staff as needed for the collective benefit of patients

**Teaching & Training**:

- Participate in teaching and training
- Participate in the induction of new staff, students and other attached professionals as necessary
- Receive review and appraisal within the PCN, and training as necessary to update skills and knowledge
