**Assessment Support Worker**
**Location: Dundee (travel will be required throughout the area)**
**Hours**:Part-time (16 or 24 hours per week) or Full-time (35 hours per week)**
**Contract type: Permanent**
**Salary: £19,838 - £20,251 per annum.** **PVG expenses paid.**

Our new innovative Discharge to Access Service is transforming the way patients transition from hospital care to their homes. Our goal is to enhance the quality of life for individuals by facilitating a smooth transition from hospital to home.

**A day in the life of an Assessment Support Worker will involve**:

- Providing support and resources to empower people to regain their independence and confidence whilst maintaining their dignity.
- Creating care plans tailored to service users needs and goals.
- Working with healthcare professionals, social workers, and other team members to ensure a comprehensive and coordinated approach to care.
- Monitor and evaluate individuals progress, adjusting care plans as needed to promote optimal recovery and well-being.
- Maintain accurate and thorough documentation of interactions, care plans, and progress.
- Assisting and enabling individuals with activities of daily living, including bathing, dressing, meal preparation medication management, and mobility support.

**To be a successful Assessment Support Worker**:

- Do you have a full UK manual driving licence?
- Do you have a good knowledge of services provided by the NHS and Social Care?
- Are you able to make things great? You know how to improve service quality for the benefit of users.
- Can you act in a professional manner, dealing with queries diplomatically and confidentially?

This role requires flexible working as it can have fast paced changes.

You could be working with different service users across your shift or working with one service user for extended periods of time.

Shift patterns include: 8am-2pm, 9am-3pm, 2pm-8pm,3pm-9pm and nightshifts are 8pm-8am and 9pm-9am.
