**Service Support Worker - Regulated Service**
**Location: Community Work, based at **British Red Cross, Dundee Technology Park, Dundee, DD2 1SW, with travel around Dundee**
**Hours: 16 or 24 per week or full-time 35 per week available**
**Contract type: Permanent**
**Salary: £21,840 per annum pro-rata for roles under 35 hours**
**Driving Requirement**:A full driving license and access to a vehicle is preferred but not mandatory**

**A day in the life of a Assist to Discharge Support Worker will include**:
- Flexible working and fast paced changes.
- Playing a vital role in providing practical and emotional assistance to individuals in their own homes assessing their needs
- Working closely with people, their families and other professionals to create care plans tailored to their needs and goals.
- Assisting and enabling individuals with activities of daily living, including bathing, dressing, meal preparation medication management, and mobility support.
- Collaborating with healthcare professionals, social workers, and other team members to ensure a comprehensive and coordinated approach to care.
- Monitoring and evaluate individuals progress, adjusting care plans as needed to promote optimal recovery and well-being.
- Maintaining accurate and thorough documentation of interactions, care plans, and progress.
**To be a successful Assist to Discharge Support Worker you'll need**:
- Previous experience in a similar care, preferably within a healthcare or social care setting.
- Compassionate and empathetic nature, with a strong desire to make a positive impact on individuals’ lives.
- Excellent communication and interpersonal skills, with the ability to work effectively within a multidisciplinary team.
- Knowledge of person-centred care principles and a commitment to upholding dignity and autonomy.
- Ability to work independently, demonstrating initiative and reliability.
- Flexibility to work various shifts, including evenings, weekends, and bank holidays.
- A full driving license and access to a vehicle is preferred but not mandatory.
