Join our dynamic team as an Assessment Support Worker in our new innovative Discharge to Access Service Were seeking compassionate and dedicated individuals to help us revolutionise the way patients transition from hospital care to their homes. This exciting opportunity offers a chance to be part of a cutting-edge approach that priorities patient-centred care and maximises independence.
This can include working with different service users across your shift or working with one service user for extended periods of time - Patterns include shifts - 8am-2pm. 9am-3pm, 2pm-8pm, 3pm-9pm and Nightshifts 8pm-8am and 9pm-9am.

**A day in the life of an**
**Assessment**
**Support Worker will include**:
- Work closely with people, their families and other professionals to create care plans tailored to their needs and goals.
- Assist and enabling individuals with activities of daily living, including bathing, dressing, meal preparation medication management, and mobility support.
- Collaborate with healthcare professionals, social workers, and other team members to ensure a comprehensive and coordinated approach to care.
- Monitor and evaluate individuals progress, adjusting care plans as needed to promote optimal recovery and well-being.
- Maintain accurate and thorough documentation of interactions, care plans, and progress.
**To be a successful Assessment Support Worker, you'll need**:
- Previous experience in a similar care, preferably within a healthcare or social care setting.
- Compassionate and empathetic nature, with a strong desire to make a positive impact on individuals lives.
- Excellent communication and interpersonal skills, with the ability to work effectively within a multidisciplinary team.
- Knowledge of person-centred care principles and a commitment to upholding dignity and autonomy.
- Ability to work independently, demonstrating initiative and reliability.
- Flexibility to work various shifts, including evenings, weekends, and bank holidays.
- A full driving license and access to a vehicle is preferred but not mandatory.
