**About the Service / Department**
Our Hounslow LIFE service provision in Hounslow includes a large integrated support service providing outreach support, advocacy and carers short breaks to people with a range of support needs; accommodation-based support to people with mental ill health or complex needs; a day service providing recovery focused and co-produced support to people with mental ill health needs, and a Hospital Discharge House that supports people without appropriate accommodation who need to move on from hospital.

All services within Hounslow work closely with local statutory and voluntary agencies taking a partnership approach towards supporting service users.

This role will lead the service by ensuring its continuing relevance to service users' needs and commissioner's and funders' requirements by ongoing planning, monitoring, evaluating and promoting the service and it's effective delivery service delivery in line with the contract, specification and Hestia's submission for delivery.

**About the role**

Our Team Leaders are responsible for supporting staff in developing, supporting and maintaining independent living skills and general wellbeing whilst maximizing independence for vulnerable service users within Hounslow LIFE. They also need to ensure that staff can use organisational safety and aspiration-based assessment tools with the service user to gain a clear understanding of their individual situation.

Other responsibilities include:

- Ensuring that staff are competent to assess and support service users with a range of needs, safety issues and strength and aspirations including mental health, substance misuse, those with multiple and complex needs.
- Ensure that staff can formulate, implement and review personalised, client-focused SMART support plans with service users and associated professional participation to develop, review and achieve goals and aspirations.
- Ensure that staff are engaging service users' supporters in their support plans (with consent) including families, carers, statutory and voluntary sector providers.
- To review that floating support staff are utilising key-working sessions with key clients to enable them to develop and sustain independence, maintain their accommodation and enjoy a quality of life.
- To ensure that staff are enabling the service users to take responsibility for their health needs and wellbeing and accessing specialist services.
- To work with floating support workers to support service users to maximise their financial security and independence through addressing arrears, maintaining benefits claims and accessing employment, training and volunteering opportunities to progress into paid employment.
- To achieve agreed standards of service delivery in compliance with relevant legislation, contractual requirements, and Hestia policies and procedures.
- To meet regularly with peer Managers and the Service Manager to discuss the delivery of the service and to raise any concerns about staffing or delivery with the Service Manager through a range of ways including both formal and informal mechanisms.
- To support service managers to meet reporting requirements.
- To undertake line management responsibilities for Support Workers, Agency Workers and Pool Workers within floating support service.

**About you**
- Team management experience including recruitment, induction, supervision, appraisal, training, performance management, discipline and grievance processes
- Proven Track record and good research skills, knowledge of key factors affecting health and wellbeing in older population and the skills and experience to translate into a clear vision, service development and delivery within specific community services.
- Able to develop professional working relationships with a wide variety of stakeholders; including community services, customers, professionals, from statutory and voluntary organisations and members of the public
- Demonstrable experience of performance monitoring to ensure a consistently high quality service which adequately meets the needs of individual service users
- Ability to communicate to a high standard, both verbally and in writing
- Experience in actively monitoring the health, wellbeing, safety and security of the services and its users ensuring all concerns are followed up promptly and in line with procedures
- Knowledge and understanding of safeguarding issues and ability to address them appropriately
