An exciting opportunity has arisen for a part time Health Liaison and Diversion Facilitator within our Liaison and Diversion Team based between the 2 custody suites in Blacon and Middlewich.

The successful applicant will be expected to work across Urgent Care Mental Health First Response Services, which will include Crisis Resolution Home Treatment Teams, Liaison Services, Street Triage Services, Liaison and Diversion Services and the Crisis Line.

The Health, Liaison and Diversion Facilitator (HLDF) will function at a high competency level. They will have responsibility for actively participating as a key member of the multi-disciplinary team within the Cheshire Integrated Health and Liaison and Diversion Service.

The HLDF will have the responsibility for providing quality health care to individuals in contact with criminal justice agencies. They will work to identify physical health concerns and mental vulnerability in police custody and court settings providing support with practical concerns or difficulties that might be affecting mental health and wellbeing; supporting individuals to access the professionally qualified team members when clinically appropriate.

The post holder will develop, enhance and improve their knowledge of the interface between criminal justice and health and social care agencies, developing skills and competencies at all opportunities.

CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care - including the provision of three GP surgeries.

All new clinical support staff will be required to have completed or working towards the Care Certificate (CWP In-house 2 day training to be completed within 6 months of start date).

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. You are encouraged to enrol for the DBS Update Service. A small annual fee of £13 per year applies.

New entrants to the NHS will commence on the minimum of the scale stated above.
