The relocation package is for anyone joining the Trust in a Clinical role who would need to relocate in order to take up employment with Cheshire and Wirral Partnership.

The Specialist Health Facilitator post is based with the Adult CLDT in West Cheshire.

The CLDT is an established team comprising of nurses including health facilitator, speech and language therapists, occupational therapists, physiotherapists, psychologists and psychiatry.

The Specialist Health Facilitator will work as part of the multi-disciplinary community support team, and will be expected to work autonomously, managing a defined case load of adult service users with learning disabilities and complex health needs.

Also they will support the leadership role around the physical health and health facilitation/ health promotion work across the team, including strategic work needing to be done to develop the service further.

There are two bases used by this team, one being in Ellesmere Port (Workplace Hub) and one being in Winsford (Wyvern House), and the location of the base will be discussed with the successful applicant.

The post holder will work as a member of the specialist multi-disciplinary team, carrying out assessments and providing effective treatment, care and support to service users who may be experiencing health problems in addition to their learning disability, ensuring those with additional complex needs are enabled to access healthcare to effectively and appropriately meet their needs via the provision of reasonable adjustments.

The post holder will be expected to work autonomously, managing a defined caseload of adult service users with learning disabilities and complex health needs, where a high level of clinical expertise is required.

The post holder will work in close collaboration with stakeholders to identify and effectively respond to the health needs of people with learning disabilities in order to improve health outcomes and enhance the experience of healthcare services for service users and their families and carers.

The post holder with support the other members of the CLDT to develop skills and knowledge around supporting service users to access the generic services they need to meet their health needs.

The post holder duties may vary to support the needs of the service.

CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care - including the provision of three GP surgeries.
