This London Borough work in partnership with Oxleas NHS Foundation Trust provides a social work service for residents within fully integrated multi-disciplinary teams. They are looking to recruit an enthusiastic and dynamic **Senior Social Worker** to join the **Locality Adult Mental Health Team**.

As a Social Worker based within a multidisciplinary team you will be working with adults over 18 living with mental health conditions, undertaking Care Act Assessments and Care Plan reviews and act as a care coordinator. High priority is given to workingtogether with service users, families and carers to enhance the independence and quality of life of people with mental health conditions.

Oxleas NHS Foundation Trust is a well
- regarded Health Trust rated 'Good' in our last CQC inspection. The service is currently arranged into Primary Care Plus, which works with GP practices to maintain patient care within primary care and to aid the smoothtransition between primary and secondary care,
