The Children and Young People’s Division are looking to recruit Shared Care Coordinator/TYA MDT Coordinator

The postholder will be responsible for the oversight and management, of the shared care services. The role incorporates, coordinate numerous MDTs within Children and Young People’s Cancer Services, assisting clinical staff by coordinating patient care, ensuring services are delivered in a timely, efficient, and robust manner.

A key aspect of the role involves regular communication with healthcare professionals across the cancer network, patients, relatives, and carers.

The post holder will act as one of the line managers for the Macmillan Cancer Centre Outpatient and ambulatory services, supporting other team leaders with Patient Target List (PTL) and Epic reports.
