Clinical Operational Lead Neighbourhood Matron

An exciting opportunity has arisen for Clinical Neighbourhood Matrons to work within Community Services. Various hours up to full/part time considered. Covering North Northumberland and South East/central Northumberland District Nursing teams. Bases to be discussed at interview.
To join Northumberland’s dynamic Community Nursing Service working across a cluster of practices within a defined geographical area. The post holder will provide high-quality working practice, while working within defined procedures and protocols as laid down by the Trust.
The Community Clinical Neighbourhood Matron will have overall responsibility for identification of vulnerable patients who are at risk and also those frequently admitted to hospital by proactive case management working with Specialist Nurses, Medical colleagues, Palliative Care, Intermediate Care, Social Care and AHP’s. The aim is to reduce admissions and improve quality of life. In addition, the post holder will lead and be responsible for District Nursing teams within the neighbourhood by developing new effective ways of working.
The Clinical Neighbourhood Matron role models compassionate and inclusive leadership in order to shape the creation of a collective leadership culture within the Trust demonstrating the creation of a collective leadership culture within the trust. This means demonstrating a consistent leadership style, engaging, enabling and empowering others, promoting ownership of learning and quality improvement.
The post holder will be responsible for operational management and clinical leadership across a primary care network working directly to the Senior Manager, maintaining a high standard of care delivery to patients, clients and relatives.
The post holder will be required to represent the Community Nursing service, working with other health and social care providers to shape
an integrated whole system approach to service delivery across the primary care network.
The post holder will work towards advanced professional practice including advanced clinical skills, medicines management, leadership, supporting self-care management and forge interagency and partnership working to improve outcomes for patients with co-existing long term conditions. In addition, the post holder will work proactively to reduce unplanned hospital
admissions and facilitate earlier discharge from hospital.
We manage three major locality hospitals at North Tyneside, Wansbeck and Hexham, plus a number of smaller community hospitals and clinics from Tynemouth to Berwick on Tweed, in addition to our state-of-the-art Northumbria Specialist Emergency Care Hospital, the first of its kind in England. We also care for people in their homes and provide services from facilities in local communities such as health centres. The post holder requires a high level of communication and interpersonal skills in order to provide effective leadership and strategic direction, facilitate change and foster conducive working relationships within services, the Trust and partner organisations. The post holder will be required to maintain good working relationships and communication links with a variety of key stakeholders. They will be responsible for chairing meetings/professional forums, for example where issues
presented are likely to be challenged.
