Location: situated at Farnham Road Practice, but travel to all sites based in Slough will be required

Farnham Road Practice is a large, progressive and innovative training Practice looking after nearly 38,000 patients with its main base on the northern edge of the diverse and vibrant Slough community, complemented by smaller sites based in eastern and southern Slough.

Our Primary Health Care Team continues to grow with GPs working alongside Practice Nurses, Health Care Assistants, Physician Associates, Clinical Pharmacists, Paramedics, MSK Practitioners and Care Co-ordinators, to provide an evolving healthcare and medical service to meet the varying needs of our patients and, in turn, provide what we believe is a sustainable model of modern General Practice.

We place an emphasis on "Putting Patients First" through team-work, communication, continual development, incident recording, team problem sharing and solving, training and education.
- Assist QOF and Enhanced Services Lead for the Farnham Road Medical Group of practices in various aspects of searches and claims, QOF admin and enhanced services.
- Work with GPs and MDT team within practices of Farnham Road Medical Group to assist in the ongoing QOF proactive care of special groups of patients across all QOF domains.
- Take active part in QOF and clinical administration of Childhood Immunisations and Vaccination programs across the Farnham Road Medical Group of practices.
- Assist the Reception team in booking QOF appointments and managing QOF appointment book.
- Assist the Docman Workflow pathway.
- Assist the Blood pressure and health checks in surgeries within the FRMG
- Work closely with the Clinical Pharmacist team in optimising QOF and Prescribing indicators along with achieving Quality prescribing.
- Build relationships with key staff in GP practices within the local Primary Care Network (PCN), attending relevant meetings, becoming part of the wider network team.
- Be part of the practice teams for anticipatory care planning and attend Integrated care team meetings as part of the regular cluster catch ups within the network.
- Be proactive in developing strong links with all local agencies to encourage referrals, recognising what they need to be confident in the service to make appropriate referrals.
- Work in partnership with all local agencies to raise awareness of social prescribing and how partnership working can reduce pressure on statutory services, improve health outcomes and enable a holistic approach to care.
- Be proactive in encouraging self-referrals and connecting with all local communities, particularly those communities that statutory agencies may find hard to reach.
- To work as part of the practice Multi-Disciplinary Team and receive and share information within that team to safeguard individuals and support them to work towards their goals and aspirations.
- To maintain accurate information systems of records and activities, complete data sheets and monitoring and evaluating data.
- To organise and facilitate group-based workshops and activities, ensuring self-help and peer support groups cover a flexible timetable.
- To support service design, co-production, development and improvement ensuring quality is maintained throughout.
- To manage a caseload of individuals with complex needs.
- To develop appropriate resources and materials for the service.

**Person Specification**

Essential
- Excellent interpersonal and communication skills both verbal and written.
- Attention to detail.
- Excellent keyboard and computer skills.
- Ability to work as part of a team and under own supervision.
- Good standard of literacy and numeracy.
- Excellent timekeeping.

**Qualifications**

Essential
- NVQ3 Qualification and
- Evidence of Medical Terminology Training and/or
- HCA or equivalent Nursing or Pharmacy services training.

Desirable
- Level 5 qualification (i.e. Diploma of higher education Diploma of further education Foundation degree HND or equivalent professional experience) training

**Experience**

Essential
- Previous knowledge of Emis, Docman & Accurx
- Previous training and experience in QOF searches and enhanced services claims
- Experience of managing a caseload of clients and keeping up to date records using a database.
- Experience of working with individuals (1-2-1) and groups of people in different settings to help them achieve their goals.
- Experience of working with a range of agencies and organisations to develop effective working relationships.
- Experience working with people with multiple needs.

Desirable
- Experience of working directly in a community development
