Primary Mental Health Practitioner

An exciting opportunity has arisen for an experienced Registered Mental Health practitioner to join the Flying Start Health Visiting Team in Wrexham. The team, which includes Health Visitors, Speech and Language therapists, Mental Health Practitioners and Nursery Nurses, provide intensive interventions for parents/carers of children from pre-birth to 4 yrs. The aim is to improve the health and well-being of the child and support and enable parents to have positive relationships with their children. As a member of the team, you will make a significant contribution by assessing, developing and implementing care plans to address and improve the mental health of the parents involved.
You will have exceptional communication skills and the ability to build and maintain effective working relationships, demonstrating a ‘can do’ attitude’.
You must be able to travel between service users homes both within towns and rural areas in a timely manner.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Responsible for carrying out comprehensive mental health assessments with service users in the community. Identify service user needs and make recommendations for services needed.
Provide evidence based therapeutic interventions for people presenting with common mental health problems, consistent with NMC Codes of Practice or appropriate governing body, and in line with NSF guidelines and Health Board policies and procedures.
Assess, Plan and Provide relevant interventions in the treatment of service user who meet the Primary Care criteria, undertaking principles of cognitive behavioural therapy, depending on the individual needs of service user.
