To be lead admin support for the Liver and Hepatitis Clinical Nurse Specialist team and facilitate in the delivery of a high standard of support to patients with liver conditions.
- Collaborate and communicate with the Liver & Hepatitis Team to ensure the provision of appropriate services to maximise the health and quality of life of the patient, improving the quality and efficiency of health care delivery.
- Triaging helpline enquires to the appropriate personnel/department within the hospital or community setting. Actively participate in the provision of routine information and advice, for patients and carers over the telephone, liaising with the CNS as appropriate. The post holder will improve patient experience by providing a friendly nonclinical contact point for patients and their families, providing support and information throughout
- Assisting the Liver & Hepatitis team in the co-ordination of patient care by: liaising with other departments, to ensure appointments and investigations are booked, booking appointments as required using EPR, liaising with patients and carers/family as required to ensure they are aware of upcoming appointments and investigation.

- Communicate with key departments (diagnostics, endoscopy, clinic bookings etc.) to: - Ensure all tests and diagnostic investigations are booked
- Completed in a timely manner and any issues are escalated.
- Communicate regularly with patients to ensure that they are aware of upcoming appointments, diagnostic tests/investigations and offer practical support with the arrangements.
- Assist with the booking of the Hepatocellular carcinoma surveillance programme, maintenance and upkeep of the database.
- To be a friendly single point of contact for patients, carers, and the healthcare team - triaging sensitive enquiries and referring to the appropriate department /clinical colleague within the service.
- To ensure that the provisions of appropriate services at the right time and in the right place are available for patients and to have the knowledge to signpost patients as required.
- Actively participate in the provision of routine non-clinical information and advice for patients and carers, helping to reduce anxiety and provide reassurance and information as appropriate.
- Develop positive working relationships with all members of the team, liaising with management, consultants, specialist nurses and administrative staff etc.
- Liaise closely with the site-specific MDT Coordinators, Network Coordinator for the Hepatitis Operational delivery Network and Hep C trust Peers Team, to ensure appropriate co-ordination and transfer of care between different departments and organisations.
- Work with the Liver and Hepatitis team to facilitate the provision of the personalised care agenda including holistic needs.
- Liaise with community teams as required on behalf of the Liver and Hepatitis specialists, this includes GP surgeries, Drug and Alcohol teams, D/N teams, community palliative care teams etc
- Support the Liver and Hepatitis team in onward referrals to other service as required, this could be D/N’s, MacMillan cancer information teams, Local benefit support teams etc
- Assist with the development of databases to monitor patient activity to inform audit.
- Assist the CNS team with monitoring patient experience
- Assist the Liver & Hepatitis Team with the organisation of annual business meetings including minute taking.
- To manage own workload, with the ability to work independently and take the initiative where appropriate, planning and organising his/her own time efficiently and effectively to meet all requirements in a timely and accurate manner.
- Triage incoming calls and initiate appropriate response according to protocols and individual pathways, liaising with the CNS as appropriate when non routine
- Utilising good communication skills, basic clinical awareness and appropriate tools, procedures, to ensure efficient case management
- Awareness that communication with patients may be impaired by stress and anxiety due to psychological burden of living with chronic/life limiting illness
- Ability to communicate in empathic and reassuring manner
- Coordinate community resources as needed
- Ensure relevant patient information is available and sent to patients as required, or available for clinics.
