**Our Adult Social Care Provider Resource aims to support people with a range of disabilities, to access a variety of pro-active enablement functions, to help maximise skills, independence and their wellbeing.**

**We are seeking to recruit an Enablement & Independence Officer, to complement our existing Outreach Service.**

Working as part of a professional and pro-active Outreach Service, you will be required to enable and empower individuals from the age of 16 onwards, to develop, maintain and enhance their enablement and independent living skills through;
- Planning, designing and implementing a number of enablement modules and programmes that focus on achievable and realistic outcomes for those customers on your caseload
- Contributing to the ongoing assessment of the customer and identifying and implementing recommendations to ensure outcome focussed objectives are achieved.
- Using effective communication skills to liaise with a range of internal and external stakeholders and professionals attend multi-disciplinary meetings as and when required, utilising a strength based approach at all times
