We have an exciting opportunity within Central London Community Health (CLCH) for anyone interested in looking for a new challenge This is an opportunity to be part of services and pathways for patients with Post Covid Syndrome (Long Covid).

The Post Covid Multi-Disciplinary Team for the Inner London boroughs of K&C, Westminster and H&F will consist of Occupational Therapists, Physiotherapists, a Psychologist and Assistant Practitioners. (All of varying grades). The team will provide assessment, intervention, support, and case management for this cohort of people in the community.

The team will build links with the acute hospital assessment clinics, community providers as well as community and voluntary sector resources. You could be part of a team whose aims are to help patients access interventions and education from different sources. You would be helping manage patients via the Living With Post Covid app and other virtual group interventions.

As an Assistant Practitioner within the Post-Covid Recovery Service you will get the chance to work with and under guidance of a qualified Allied Health Professional (AHP) supporting service users, their families, friends and carers. You will undoubtedly play an important role in how they experience our service.

We are looking to recruit 1x Full-Time, Band 4 Assistant Practitioners. The main base will be Soho Centre for Health and Care W1, with options available for hybrid working. The service operates between the hours of 9.00-17.00 Monday - Friday.

To complement & provide continuity of the work of the qualified AHPs across the team and thus being able to carry your own caseload for follow up treatments.

Under the direction of a qualified AHP, participate in Triage by collecting information pertinent information to support the assessment and decision making, prepare the referral for MDT triage.

Under the direction of a qualified AHP, independently carry out individual patient rehabilitation assessment and treatment plans and group programmes thus carrying and encourage consistent engagement.

To take patient’s observations / vital signs and use patient reported outcome measures (PROMS) as indicated in order to report on patient progress to the treating AHP and /or the MDT forum.

To provide continuous assessment of environment and potential safety risks.
