We are looking for an energetic, highly motivated Psychological Wellbeing Practitioner / Assistant Psychologist to join our Diabetes Psychology Team. We need someone who is enthusiastic and motivated, who would like to provide evidence based psychological interventions for people living with diabetes. You must have experience in working with people with diabetes or another long-term health condition.

The Psychological Wellbeing Practitioner / Assistant Psychologist role involves working in a diabetes service to help facilitate and improve care pathways for people with mental health needs and diabetes. The post holder will work with the Hackney Diabetes Centre Psychology Team in improving engagement, self-management and outcomes for people living with type 1 diabetes. They will also work specifically in identifying and working with patients who have not engaged with the wider diabetes service. The post holder will also provide one to one and group-based support to individuals struggling with common mental health problems.

The role of the Psychological Wellbeing Practitioner/ Psychology Assistant will also complement the existing psychology service based in the diabetes service.

**Duties include**:

- To identify and assess patients struggling to engage with services
- To assess patients referred to the service under, supervision, to identify their psychological needs
- Provide signposting and treatment including one to one guided self-help using the services treatment protocols, Computerised CBT, bibliotherapy and through delivering the services group based interventions at step 2
- To ensure that the work is patient focused and responsive to need, with particular reference to the needs of black and other minority ethnic communities.
- To work closely with other team members to ensure the service is effective and collaborative
- To accept and engage with supervision arrangements, both clinical and managerial
- Contribute to the identification of local mental health needs, gaps in service provision, and any other relevant issues
- To provide monitoring information concerning the contacts, outputs and outcomes as required
- Undertake audits where identified
- Develop new appropriate resources for use by team and related professional
