A CLINICAL LEAD IS REQUIRED FOR THE NEWLY ESTABLISHED DISCHARGE TO ASSESS TEAM WITHIN LANCASHIRE & SOUTH CUMBRIA ICB
YOU WILL BE REQUIRED TO MANAGE A TEAM OF STAFF WHO COVER THE NUMEROUS ACUTE TRUST SITES ACROSS THE LANCASHIRE& SOUTH CUMBRIA FOOTPRINT AS WELL AS LIAISING WITH PLACE BASED CLINICAL LEADS
PLACE BASES INCLUDE: BLACKPOOL, FYLDE & WYRE, MORECAMBE, SOUTH CUMBRIA, LANCASTER, BLACKBURN WITH DARWEN, GREATER PRESTON, CHORLEY & SOUTH RIBBLE, EAST LANCASHIRE & WEST LANCASHIRE
ROLE IS FAMILY FRIENDLY AND OPEN TO ALL ALLIED HEALTH PROFESSIONALS & NURSES
The post holder will play a key role in supporting the All Age Continuing Health Care service across the region.
Working within the remit of the National Framework for Continuing Healthcare, the post holder will deliver an effective and competent level of service, consistently delivering a client-focused service.
The post holder will manage a team of professionals covering Discharge to Assess process and provide specialist advice to the CHC Advisors. The post holder will have responsibility for the operational and line management duties of the CHC Team in the commissioning of care packages in the community and in residential settings. The post holder will work closely with the Head of Service CHC
The post holder will work closely with acute trust & adult social care colleagues as well as commissioning leads in the ICB and Local Authority in health developments and strategy.
