The post holder will play a key role in supporting the All Age Continuing Health Care service across the region.

Working within the remit of the National Framework for Continuing Healthcare, the post holder will deliver an effective and competent level of service, consistently delivering a client-focused service.

The post holder will manage a team of professionals covering Discharge to Assess process and provide specialist advice to the CHC Advisors. The post holder will have responsibility for the operational and line management duties of the CHC Team in the commissioning of care packages in the community and in residential settings. The post holder will work closely with the Head of Service CHC

The post holder will work closely with acute trust & adult social care colleagues as well as commissioning leads in the ICB and Local Authority in health developments and strategy.

Currently we are looking for a motivated and experienced RMN, RGN OR ALLIED HEALTH PROFESSIONAL as Clinical Lead for our newly established Discharge to Asses Team.

The post holder will play a key role in supporting the All Age Continuing Health Care & Individual Patient Activity service across the region.

The post holder will deliver an effective and competent level of service, consistently delivering a client-focused service.

The post holder will be responsible for providing management and clinical leadership and overseeing a team of professionally qualified nurses in the delivery of NHS Continuing HealthCare Service and Deputising for the Head of Service as required

The post holder will actively contribute to the strategic planning of team projects, identifying interdependencies across projects/functions, potential impacts on wider organisation, resource requirements and building in contingency and adjustments as necessary.

The post holder will work with the Business Manager to ensure delivery and compliance with key performance indicators and performance targets.

The post holder should hold a full UK driving licence and have access to a vehicle to fulfil the requirements of this post.

Lancashire & South Cumbria ICB is a fully-authorised ICB responsible for the full range of ICB statutory duties and powers. Each Place Based Team within the ICB aims is to improve the health of the people and reduce health inequalities through strong, clinically-led commissioning of high quality healthcare services.

Key Responsibilities
- Responsible for the leadership, management & oversight of the assigned CHC Team within ICB working alongside other leads within both CHC and Complex/IPA teams
- Responsible for ensuring that all clinical aspects of enquiries, disputes, challenges and complaints are managed and co-ordinated in a timely and appropriate manner in line with policy and guidance.
- Lead the team in the delivery of project plans, allocating tasks as appropriate, identifying risks, issues and dependencies, considering best practice and current options and ultimately making decisions in the best interest of the project
- Providing management and clinical leadership
- Overseeing a team of professionals in the delivery of NHS Continuing HealthCare Service and Deputising for the CHC Head of Service as required
- Work with the Business Manager & Business Support Team to ensure delivery and compliance with key performance indicators and performance targets
- Actively contribute to the strategic planning of team projects, identifying interdependencies across projects/functions, potential impacts on wider organisation, resource requirements and building in contingency and adjustments as necessary
- Liaise with other clinical leads to ascertain themes & trends across LSC ICB and place
- Provide weekly updates to Heads of Service regarding theme, trends, issues and plans implemented to manage and resolve these

Communication and Relationship Skills
