Job Summary:
Care coordinators play an important role within practices to proactively identify and work with people, including the frail/elderly and those with long-term conditions to provide coordination and navigation of care and support across the practice, PCN and health and care services. They will work closely with the Advanced Clinical Practitioners, GPs and other primary care professionals within the practice and PCN to identify and manage a caseload of identified patients, making sure that appropriate support is made available to the and their carers, and ensuring that their changing needs are addressed. This is achieved by bringing together all the information about a person’s identified care and support needs and exploring options to meet these within a single personalised care and support plan, based on what matters to the person.

The purpose of the role is to:

- Offer general assistance to the acute care team, and wider practice team and project a positive and friendly image to patients and other visitors, either in person or via the other communication channels, including telephone and digital channels.
- Receive, assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way.
- Undertake a variety of administration duties to assist in the smooth running of the practice, including the provision of admin support to clinical staff and other members of the practice team.
- Facilitate effective communication between patients, members of the primary health care team, secondary care and other associated healthcare agencies.
- Ensure that all incoming and internal patient correspondence is scanned and stored on/in the patients’ medical record and relevant information is coded.

**Duties and responsibilities**

The duties and responsibilities to be undertaken by members of the practice administration team may included any or all of the items in the following list. Duties may be varied from time to time under the direction of the Practice Manager, dependent on current and evolving practice workload and staffing levels.

**Acute team tasks**:
Including but not limited to
- Opening up/locking up of practice premises and maintaining security in accordance with practice protocols.
- Maintaining and monitoring the practice appointments system.
- Processing Patient Triage for appointments, visits and telephone consultations and ensuring patients are directed to the appropriate healthcare professional.
- To work in accordance of written protocols.
- Taking accurate messages and passing on information.
- Filing and retrieving paperwork.
- Computer data entry/data allocation and collation, processing and recording information in accordance with practice procedures.
- Initiating contact with and responding to requests from patients, other team members and associated healthcare agencies and providers.
- Undertake any other tasks appropriate to the post requested by the partners or managers.
- Processing and sending urgent/same day patient referrals using DRSS.
- Coding QoF information such as smoking status, ethnicity etc.
- Utilising the Pharmacy First referral scheme.

**Administration responsibilities**:
Including but not limited to:

- Ensure correspondence, reports, results etc are filed in correct patient records.
- Add pathology requests to ICE.
- Support and guide members of staff in the use of clinical system(s).
- Ensure security of data at all times.
- Undertake any other tasks appropriately to the post requested by the partners or managers.

- In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately.
- In the performance of the duties outlined in this job description, the post-holder may have access to confidential information relating to patients and their carers, practice staff and other healthcare workers. They may also have access to information relating to the practice as a business organisation. All such information from any source is to be regarded as strictly confidential.
- Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the practice policies and procedures relating to confidentiality and the protection of personal and sensitive data.

**Health & Safety**:
