Key Responsibilities Responsibility for a defined caseload of patients and will provide an effective and high quality patient-focused service that is sensitive to the needs of users and providers Organises, prioritises and evaluates workload based on accepted referral criteria in line with operational protocols Puts quality at the heart of practice by delivering evidence based individualised and personalised care through holistic needs assessment, planning and evaluation of care and all care interventions to meet the needs of palliative care patients including symptom control and complex psychological issues. Will have responsibility for leading identified and agreed aspects of the service, in the development of protocols and guidelines working as part of a multi-disciplinary team and developing excellent interdepartmental relationships with excellent communication with a focus on quality improvement. Will have responsibility for the delivery of specialist nursing skills within complex pathways, provides clinical expertise, specialist advise and support across service boundaries within palliative care. Responsible for providing evidenced based practice Will ensure cost effective use of all resources and appropriate governance management.

Work both autonomously and as part of the team, to provide a resource of specialist advise, information and support to all members of the multidisciplinary team in the delivery of palliative care Defines a clear pathway by effective liaison between all carers involved with palliative care patients, in the community setting, hospital, hospice, care homes by the establishment of identifiable communication channels, ensuring a seamless service. Work within national policy frameworks relevant to the service and trust local guidance. Demonstrates a high level of empathic and interpersonal communication skills for palliative care patients and their carers
- advance care planning, including breaking bad news and end of life decision making. Deal effectively and professionally with patient/carer user issues collaborating with PALS as appropriate and take a proactive approach to resolving issues and learning from complaints.

Contribute and lead the multi-professional approach in the management of patients in relation to ongoing care needs Ensure effective multi-disciplinary communication Provide mentorship and preceptorship as required to all staff in the team or students on placement. Work closely with the lead nurse in specialist palliative care to write departmental policies and lead in the delivery of local and national guidelines. Responsible with the lead nurse for managing staff capacity and service for effective clinical cover Promotes the service and clinical excellence by publishing and presenting innovations, audit and research locally and nationally Work as an independent non
- medical prescriber within specialist palliative care taking responsibility for prescribing a range of drugs within area of competence in accordance with MYHT medicines management policy and BNF/PCF Complete annual declaration of intention to practice non medical prescribing in accordance with MYHT non
- medical prescribing governance framework Ensure appropriate skill mix, cover is provided when planning rotas and delegating workload. Delegate work to match capabilities and workload of team members.

Monitor sickness, absence and competence issues in line with Trust Policy. Lead recruitment and selection of staff in line with Trust Policy.
