- disciplinary communication. Leads on EoL mandatory training, and act as a resource in relation to end of life care issues. To be aware of local and national public health agenda, contributing positively to achieving targets that will reduce inequalities and improve health outcomes for patients and their families.

Demonstrate a working knowledge of Trust and Professional Nursing policies and procedures and adopt and advise on these accordingly. Lead for implementing the Gold Standards Framework and delivering training packages in collaboration with the Specialist Palliative Care team. Evaluate the impact of education and service development initiatives, against aims and objectives in Education and training strategy to ensure the programme is transforming the end of life care experience for patients, carers and their families. Support clinical teams by acting as a resource to ensure symptoms are optimally managed and anticipatory medication considered and prescribed in line with local prescribing guidelines.

Promote evidence based practice with measurable standards and outcomes to enhance care, reduce morbidity and improve quality end of life care for patients Act as a resource for professionals seeking advice on managing a patients symptoms at the end of life, for complex issues seek the advice from senior colleagues within the specialist palliative care team. Provide leadership to the EoL care roaming team, fast track discharge facilitator and to deputIse in the absence of Lead Nurse Palliative Care when required.
