- Knowledge of health and social care services in the community

**Role responsibilities**:

- Ensure effective two-way communication with members of the inpatient and community MDT so that patients receive timely optimum care.
- Work as an integral part of the MDT across the wards and participate in patient assessments where required.
- Maintain records in accordance with the Trust’s policy and that of the relevant professional body’s Standards for Records and Record keeping.
- Participate in audit programmes and data collection appropriate to the role.
- Identify the need for and coordinate multi-disciplinary team meetings/case conferences and support ward staff in the decision making processes with regard to the safe, effective and timely discharge of individual patients.
- Promote and support patient advocacy, choice, self-empowerment and encourage patient independence, ensuring full engagement in decisions regarding their future care needs.
- Participate in discharge planning meetings concerning the management of patients with complex needs
